“Tortured by his lust for two women”

This is a 1975 Italian film set mostly in one basement, where Emanuelle (Rosemarie Lindt) has chained up Carlo (George Eastman), the sadistic lover her sister escaped before taking her own life. She drugs him and forces him to watch her have sex, and under the drug he starts to see bloody, cannibal horrors. Joe D'Amato keeps the camera inside the cellar with them.
